The following restaurant inspections with critical violations were conducted by Richland Public Health between Feb. 28 and March 3:
● Mansfield/Ontario Fairfield Inn & Suites, 1065 N. Lexington-Springmill Road, Mansfield, Feb. 28. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed opened bags of boiled eggs, turkey sausage, and pork sausage not date marked. PIC verified when product was opened and placed date mark on items during inspection. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed spray bottle of cleaner in kitchen are not labeled. Improper storage of poisonous or toxic materials (critical, corrected). Observed spray bottle of kitchen cleaner stored hanging on side of metal storage racks for clean equipment and over a box of coffee grounds. Food display not properly protected from contamination by consumers (critical, corrected). Observed apples in customer self-service buffet area not protected from contamination. Employee pulled from sale area.
● Comfrey Corner Inc., 1145 Park Avenue West, Mansfield, March 1. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Observed no signed employee illness agreement. Improper storage of poisonous or toxic materials (critical, corrected). Observed dog shampoo stored above dog and cat food/treats.
● Party Place, 556 N. Lexington Springmill Road, Mansfield, March 2. Missing and/or incomplete allergen declaration(s) (critical). Observed multiple bulks candies without allergen declaration or ingredients in bulk self service candy area. Ensure list of ingredients and allergen declaration is available for all bulk self-service candy by March 9.
● Pumpkin Seed Bulk Food Co., 1625 Executive Drive, Mansfield, March 2. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed mozzarella, pepper jack and American cheeses with a date mark for 21 days. Soft cheeses not exempt from date marking shall be held for no more than 7 days.
● Lexington Eastern Elementary, 155 Castor Drive, Lexington, March 2. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). No bleach sanitizer was detected in wiping cloth bucket.
● Liberty Nursing Center of Mansfield, 535 Lexington Ave., Mansfield, March 2. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed quart containers stored clean under the coffeemaker area to have visible accumulation on the food contact portion of the container. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected). Observed sanitizer buckets in the kitchen to be over 200 ppm. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed scissors that were not easily separable for cleaning being used to open food packages. Person in charge (PIC) discontinued use until a screw driver or new scissors can be obtained. Presence of rodents (critical). Observed rodent droppings in the drawers of the east dining hall cabinetry, dry storage and tray storage areas, and the back corner of the kitchen by the ovens. Clean all evidence, monitor the area, and contact a licensed pest control provider by re-inspection March 9. Food contact surfaces not easily cleanable (critical, corrected). Observed can opener blade with chips rendering the surface no longer smooth, durable, nonabsorbent and easily cleanable. Ensure can opener blade is maintained to avoid metal shaving/chip contamination. PIC discontinued use of can opener until new blade can be obtained. Have new blade by reinspection March 9.
● Mansfield/Ontario Hampton Inn, 1051 N. Lexington Springmill Road, Mansfield, March 3. Hot water for mechanical sanitization is below required temperature (critical). Observed hot water sanitize cycle on dishwasher to only reach 155 degrees. Facility to temporarily use plastic tubs to manually dip equipment and utensils in sanitizer solution until dishwasher functions properly. Correct by March 10. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed can of whipped topping store on counter in customer self-service buffet area to have internal temperature of 55 degrees. PIC discarded whipped topping. No air gap or approved backflow prevention device on the plumbing system (critical). Observed no backflow device on Bunn coffee maker in kitchen area or on juice machine in self-service buffet area. Correct by March 10.
● Primrose Community of Mansfield, 1301 Millsboro Road, Mansfield, March 3. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed a spray bottle of sanitizer at 0 ppm.
● J & J Food Mart, 305 Marion Ave., Mansfield, March 3. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). Through discussion with PIC, it was determined that facility is combining sanitizing step (chlorine) and detergent step, in the facility three-compartment sink. Ensure the wash, rinse and sanitize steps happen separate compartments of the three-compartment sink at appropriate temperatures and concentrations. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed bacon and eggs being stored over milk, fully-cooked hot dogs and juice.
